What's Up At COMET?
With help from our partners, the COMET team has been working hard to bring together the ongoing work in the community with health and human services organizations. These organizations include pediatricians, obstetricians, early childhood health care organizations, and child developmental service agencies serving the same children and families. We are improving and creating tools to:
- share referrals across platforms
- share reports on the work happening in the community
- reduce or eliminate the need to enter information in more than one software system
Improved data integration capabilities for the primary health care providers in a community are necessary for closed-loop referral tracking and reporting. These providers include Accountable Care Organizations (ACOs), Managed Care Organizations (MCOs), and Regional Health Information Organizations (RHIOs).
Initiatives like these are strengthening the health and wellbeing of children and families because of the value that prevention and early detection, combined with efficient coordination of existing service providers, allow for delivering timely services to children.
